Department of Labor Posts New FMLA Forms and Poster

The United States Department of Labor (DOL) has posted to its website the new forms and poster for use in administering leave under the Family and Medical Leave Act (FMLA) under the new regulations that go into effect January 16, 2009.  The forms are available at the DOL's webpage on the revisions to the FMLA, which you can reach by clicking here.  Links to the individual forms and poster are also set out below.  The documents are in PDF format and are downloadable and fillable.
